Thank you for choosing to participate in services offered by Spa Mack. These may include, but are not limited to:

  • Bodywork and massage

  • Movement and embodiment classes

  • Vibration plate sessions

  • Guided meditation and breathwork

  • Red light therapy

  • Halo therapy (salt therapy)

  • Infrared sauna

  • PEMF (Pulsed Electromagnetic Field) mat

  • Pilates reformer sessions

  • Cold plunge

  • Hot tub use

By Scheduling with Spa Mack, you are agreeing to the following waiver and participation in any services at Spa Mack indicates your understanding and agreement to all of the following:

1. Acknowledgement of Risk

I understand that participation in wellness services involves physical, thermal, and/or emotional activity and may carry inherent risks, including but not limited to: muscle soreness, dizziness, dehydration, blood pressure fluctuations, physical exertion, emotional release, or injury. I affirm that I am in good health and capable of participating in the services offered. I agree to inform the practitioner of any medical conditions, injuries, medications, or limitations before each session.

I understand that specific services—such as sauna use, cold plunge, PEMF, or red light therapy—may not be suitable for individuals with certain medical conditions (e.g., heart disease, pregnancy, seizures, circulatory issues, or implanted medical devices). I acknowledge it is my responsibility to consult with my healthcare provider before using these modalities.

2. Voluntary Participation

I voluntarily choose to participate in these services and take full responsibility for my physical and emotional well-being during and after my session(s).

3. Medical Disclaimer

I understand that services provided by Spa Mack are intended for wellness and relaxation and are not a substitute for licensed medical care, diagnosis, or treatment. Practitioners do not diagnose medical conditions or prescribe medication. I agree to consult with a licensed medical professional regarding any health concerns.

4. Release of Liability

In consideration for being allowed to participate in wellness services, I hereby release, waive, and hold harmless Spa Mack, its owners, employees, practitioners, and affiliates from any and all liability, claims, demands, or causes of action arising from my participation.

5. Emergency Care

In the event of a medical emergency, I authorize Spa Mack to seek appropriate emergency medical care and to contact my emergency contact below.

6 Age Requirement

Participants must be 18 years of age or older.
If under 18, a parent or legal guardian must be present and actively accompanying the minor for the entire duration of all services. No minors may participate unaccompanied.
